Chronic diseases are becoming more common amongst us and affect the way we work and live. GP Chronic Condition Management Plans (GPCCMP) are designed to improve the management of long-term conditions by providing a clear treatment pathway and facilitating multidisciplinary care.
What qualifies for a GPCCMP?
A chronic medical condition is defined as a condition that lasts for six months or more. It is a health issue that requires long-term management, often involving multiple health services, including specialists and allied health providers.
